I found a bug in 2016 where if the e-mail is in Plain Text View, the attachments do not show as being attached. For example if someone decides to forward an e-mail that arrived in plain text view, the attachments do not show up as attached. HOWEVER that attachment is indeed there if sent. I found this, and Microsoft's answer was it's a feature???? I don't think Microsoft fully understands the issue. Because this is if anything a security issue, if a user doesn't understand what they're attaching.
